Eye Benefits Information

As a member of the UVa Health Plan, you may have received an e-mail from Eye Benefits or "webmaster@eyebenefits.com", to provide information about the University's vision care benefits program and instructions for how to establish an on-line account. Unfortunately this e-mail could easily be mistaken for junk e-mail and a number of employees have reported that their e-mail was filtered into their "junk" e-mail folder. Please note that you are not required to establish an account or use the website features in order to receive benefits under this program.

As in previous years, you will be receiving a new Eye Benefits card that has been mailed to your home address. By using this card with local providers you may receive discounts on the cost of eye exams, eyeglasses, and contact lenses.

The Eye Benefits e-mail offers employees an opportunity to open an account and print an electronic version of the discount card directly from http://www.eyebenefits.com. The electronic discount card is a new feature available to members of the UVa Health Plan.

When using this website:
  • You may obtain more information about the benefits available to you when using the Eye Benefits card and review a list of local providers who accept the card.
  • You have been sent an e-mail containing a user name and password to open an account at the Eye Benefits website.
  • The electronic version of the card is available to you year round should you misplace your Eye Benefits card or wish to print additional cards for dependents covered by the University's Health Plan. To print an electronic Eye Benefits card, click on "My Account" on the bottom of the Eye Benefits home page. Then type in your username and password. This will take you to a screen called "My Account Information." Click on the tab on the left called "Membership Verification". This will bring you to a screen that prompts you to click on the "Membership Verification" button. Once you open your membership verification, simply click on the print icon on the top.
  • You may also change your address with Eye Benefits.
If you have already deleted the e-mail or have not received the e-mail from Eye Benefits, please visit http://www.eyebenefits.com. From there you may click on "My Account" at the bottom of the homepage, click on "What if I forgot my User Name or Password?", and enter your e-mail address in order to receive your login and password.

We sincerely apologize for any confusion or concern that the Eye Benefits e-mail may have caused. If you have other questions, you may contact Joanne Hayden, UVa Health Plan Ombudsman, at 924-4346 or jrh5y@virginia.edu.
