Instructions for Completing The UVa HR Faculty/Staff Directory Information Form
The UVA HR Faculty/Staff Directory Information form (click here for sample form) reflects the current data in UVA HRMS that may be printed in the University of Virginia Directory (phone book). If any of this information is inaccurate, you may use this form to indicate the correct data in the "Correction" column. Below are descriptions of the data in each field.

Organization
This is the code and name for the department or Organization in which you are employed. This cannot be changed via this form.

Reason for Submitting (Circle One): New, Remove, No Change, Changed
Circle one reason why you are submitting the form. If multiple reasons apply, circle "Changed".

Publish Home Phone # and Address?
You may elect to not have your home phone number and home address printed in the Directory by selecting "No" or leaving the field blank. If "Yes", the information will be printed in the Directory.

Prefer email rather than paper from University Human Resources
Certain communications may be sent via email or regular mail from University Human Resources. If you prefer to receive email rather than paper, select "Yes". If "No" or blank, the communications will be sent by paper.

Employee Number
This is an identifying number automatically assigned by HRMS. It cannot be changed.

Computing ID
This is the identification number assigned by Information Technology and Communication (ITC) and represents the characters immediately preceding the @ in your University email address. This cannot be changed via this form.

Last Name, First Name, Middle Name (or initial), or Name Suffix
If any of these fields are incorrect, notify your UVA HRMS Specialist who must make the correction in HRMS.

Preferred Name (Nick Name)
If you prefer a different name or nick name on your HRMS record, you may list it here.

Directory Last Name, Directory First Name, Directory Middle Name, Directory Suffix
If you wish an entirely different listing in the Directory than would be printed by using the information in the fields for Last Name, First Name, Middle Name (or initial), or Name Suffix, you can enter information in these fields.

Home Address Line 1, Home Address Line 2, Home Address Line 3, Home City, Home State, Home Zip Code, Home Telephone
This is your home address and home phone number that will be printed in the Directory unless you have selected "No" in the field "Publish Home Phone # and Address?".

Work Title
This is the working title assigned to your position. It cannot be changed via this form. If your working title is incorrect, contact your supervisor. Working titles are updated by submitting a request to HR Consultant Services through Jobs@UVA.

Directory Address Line 1, Directory Address Line 2, Directory Address Line 3, Directory City, Directory State, Directory Zip Code
This is your office address/location that will be printed in the Directory.

Department Name
This is the department name associated with your Organization. It cannot be changed via this form.

Messenger/Office Address Line 1, Messenger/Office Address Line 2, Messenger/Office Address Line 3, Messenger/Office City, Messenger/Office State, Messenger/Office Zip Code
This is your messenger mail address.

Work Telephone, Work Secondary Telephone, Work Fax, Mobile Telephone, Private Mobile, Pager, Pager Number Code, Pager Pic Number, Private Pager, Private Pager Number Code, Private Pager Pic Number, TTY Number
These represent the various telephone numbers that may be maintained in HRMS. Those not identified as "private" will be printed in the Directory. Those identified as "private" are not printed in the Directory but are on this form for verification purposes only.
