Death of an Employee Life Events Birth or Adoption Marriage Divorce Death of an Employee Continuation of Benefits (COBRA) Medical Care Out of the Network Your dependents and/or beneficiaries should call the University Human Resources Office of Employee Benefits, 924-4392, to report your death. Once the death is reported, the surviving spouse or dependents will receive detailed information on benefit continuation. There may also be paperwork to complete related to life insurance, accidental death, and retirement. The Office of Employee Benefits will assist your family members complete any necessary forms.
